Athletes Be Prepared

All student athletes need a physical, concussion form and code of conduct on file before they can practice or play. Students can turn these signed forms into the office or their coach before they start practicing.
Alegent Health Clinic is one option for high school sports physicals at a cost of $20. The Alegent Health Clinic is located at 110 N. Main Street in Oakland, Iowa (712-482-6484). They will do physicals on Tuesday, July 17th and August 7th at 4:30-6:30 PM. The $20 fee does not include immunizations or lab tests.
